A Subsidy Health and Safety inspection was conducted on December 20, 2019 to review supplemental health and safety requirements. Licensing Inspector arrived & started inspection at 10:35 PM. There were eleven (11) children ages 14 months-4 years under the direct care and guidance of four (4) staff members present. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Licensing Inspection conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Center Director on December 20, 2019. EVIDENCE:
Licensing Inspector reviewed seven (7) staff records. Records for Staff #6 and #7 did not include the date of employment.Plan of Correction: Director will document the date of employment for each staff member.
Standard #: 22VAC40-665-580-D Description: Based on record review, the Vendor failed to ensure staff members complete orientation training.

Licensing Inspector reviewed seven (7) staff records. 2 out of the 7 records reviewed were recently hired staff. Records for staff #6 and #7 did not include documentation that they have completed orientation training. Records for Staff #6 and #7 did not include the date of hire.Plan of Correction: Staff will complete center orientation training.
Standard #: 22VAC40-665-580-H Description: Based on record review & discussion with the Center Director, the Vendor failed to ensure each staff person completes at lest 16 hours of annual training.

Licensing Inspector reviewed seven (7) staff records. Record for Staff #2 did not include documentation of at least 16 hours of annual training. Staff member has completed 10 hours.Plan of Correction: Staff member will complete additional 6 hours of annual training for 2019
