An unannounced monitoring inspection was conducted on Tuesday, October 22, 2019 from 9:55 AM to 11:40 AM to examine the areas of noncompliance identified in the Intensive Plan of Correction (IPOC). Two staff records, two children?s records, and all areas addressed in the IPOC were reviewed.
A member of management was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The laundry area in classroom #3 is separated by a swinging door. The laundry swinging door was unlocked and the pad lock and latch were hanging from the swinging door and not connected to the wall.
2. In the laundry area, one bottle of laundry detergent labeled "Keep out of reach of children" and "May be harmful if swallowed", one bottle of bleach labeled "Keep out of reach of children" and "Danger", and one container of FOG solution labeled "Keep out of reach of children" were not kept in a locked place using a safe locking method that prevents access by children.
3. In classroom #3, there were three children,16 months of age, 21 months of age and 20 months of age.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center did not ensure that diapering standards were followed.
Evidence:
1. The diaper changing pad in classroom #1 had a large tear and was no longer nonabsorbent. The standard requires a nonabsorbent surface for diapering.
2. The plastic-lined storage system in classroom #1, #2, and #3 had a broken operational foot operated pedal. The standard requires that the plastic lined storage system is either foot-operated or used in a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system during disposal.
3. The plastic-lined storage system in classroom #1, #2 and #3 had multiple soiled diapers deposited in it.Plan of Correction: Not available online. Contact Inspector for more information.
