Comments:
An unannounced complaint inspection was initiated and completed on Friday, January 10, 2020 from 11:58 AM to 1:15 PM. Interviews were conducted with staff, records were reviewed and observations of the program were made relating to the complaint report. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection. See the violation notice for more details.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
In room #1, the ceiling title was warped and exposing insulation.

Description: Based on observation, the center did not ensure that cots used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence:
In room #3, eight children did not have a bottom cover during the designated rest time.
