Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site monitoring inspection was initiated on 5/17/24 and concluded on 5/17/24. The on-site inspection began at 8:45am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 83 children present and 14 staff. The inspector reviewed five (5) children?s records and five (5) staff records on-site. This inspection included: document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Physicals are to be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

Evidence:
There was no proof of physical found for child #5, date of enrollment was 02/01/24.

Plan of Correction: A physical will be obtained and placed in the child's record.

Standard #: 8VAC20-780-150-B
Description: Proof of immunizations are required to be signed and dated by a physician, his designee, or an official of the local health department.

Evidence:
The immunization document available for child #2 was not signed and dated.

Plan of Correction: A signed and dated copy of immunizations will be obtained and placed in the child's record.

Standard #: 8VAC20-780-270-A
Description: Equipment, both inside and outside, must be maintained in a clean, safe, and operable condition.

Evidence:
On the big playground the material used under the mulch was sticking up in numerous places posing a tripping hazard to the children.

Plan of Correction: The ground covering will be fixed so there are no longer tripping hazards on the playground.

Standard #: 8VAC20-780-330-B
Description: There shall be appropriate fall zones under and around equipment with moving parts or climbing apparatus.

Evidence:
The fall zones did not meet compliance on the small and large playgrounds as there were other climbing apparatus and play structures within the fall zones of the permanent climbing equipment.

Plan of Correction: The equipment within the fall zones on both playgrounds will be moved and/or removed to ensure fall free zones as required.
