Comments:
An unannounced monitoring inspection was conducted on May 6, 2024 between the hours of approximately 9:15 a.m. and 9:55 a.m. There were 4 children present with the Provider for a total of 13 points within the supervision guidelines. The children were observed arriving to the home, engaging in free play, and preparing for a morning lesson. Positive interactions were observed during this inspection. A complete inspection of the physical plant, fire drill log, menu, attendance and emergency procedures were observed during this inspection. 1 child's record was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 business days from today. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on record review, records of emergency evacuation and shelter-in-place drills were not maintained for one year.

Evidence: At the time of inspection, the family day home did not have documentation of emergency evacuation and shelter-in-place drills.

Description: Based on observation, empty plastic bags large enough for a child's head to fit inside were accessible to children under five years of age.

Evidence: Empty plastic bags were in a storage cube, accessible to Child #2 (age: 19 months) and Child #3 (age: 3 years).

Description: Based on observation, potentially poisonous substances, materials and supplies were accessible to children.

Evidence:
1. Two containers of petroleum jelly, one bottle of baby powder and one insect repellent were in an unlocked diaper changing table drawer, accessible to children.
2. One container of disinfectant wipes were on a table, accessible to children.
