63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was completed on 08/15/2019 from 10:00 A.M. until 3:00 P.M. At the time of the inspection 11 children were in care with five staff and one administrator present. Five children's records, five staff records, required posted information, evacuation drills, health inspection, fire inspection, first aid kits and supplies, emergency plan and outside play area were reviewed. Children were observed during during outdoor gross motor time which included a walk, music and movement, storytime, an art activity, diapering, handwashing, free choice activities, lunchtime, and naptime. Violations:
Standard #: 22VAC40-185-280-E
Description: Based on observation, the center failed to store cleaning materials and insecticides/pesticides in areas physically separate from each other. Evidence: 1. In Classroom B under the sink in the classroom there was Down and Out Flying Insect Killer stored with Soft Scrub cleaner, and a container of disinfectant wipes. 2. Staff verified that there were insecticides/pesticides and cleaning supplies stored in the same cabinet.

Plan of Correction: Staff will remove the insecticides/pesticides from the classroom. Staff were not aware that the insecticides/pesticides were located in the cabinet under the sink.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure that all children hands were washed with soap and running water or disposable wipes before eating meals or snacks. Evidence: 1. In Classroom A three out of six children did not wash their hands before coming to the table for lunch. 2. In Classroom A three out of six children washed their hands after using the restroom, and then went back into the classroom and joined in free choice activities. When they came to the table for lunch they did not rewash their hands with soap and running water, or a disposable wipe before eating lunch. 3. Staff verified that the children did not wash their hands before lunch.

Plan of Correction: Administration will review hand washing requirements with all staff.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that proper diapering procedures were followed. Evidence: 1. When changing an Toddler pull-up, Staff #6 was observed disposing of a soiled pull-up. Staff #6 turned to place the pull-up in the foot operated waste can. Instead of using her foot to open the waste can lid she opened the lid with her hand and disposed of the pull-up. 2. Staff #6 verified that she did not use the foot pedal to open the waste can lid, but used her hand.

Plan of Correction: Administration will review all diapering requirements with the staff.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff records, the center failed to ensure that an out of state central registry background check was completed within thirty days of employment. Evidence: 1. Staff #5 began employment on 09/05/2018. This staff person indicated that she resided in one state other than Virginia within the past 5 years. The required out of state central registry search request for this staff person has not been submitted. 2. Staff verified that the record check had not been submitted within the required time frame.

Plan of Correction: Administration will complete an out of state central registry background check immediately.
