An investigation was initiated on 06/14/2019 from 8:15 a.m. - 10:45 a.m. and 06/19/2019 from 11:15 a.m. - 11:45 a.m. to investigate a complaint regarding ratio's and care, protection, and guidance. Interviews were conducted, documentation, and staff records were reviewed. There was not sufficient evidence to corroborate the complaint allegations therefore, the complaint regarding ratio's and care, protection, and guidance was determined to be not valid.
