Standard #: 22VAC40-185-130-A Description: Based on review of five children's records and verification from the owner, one child's record did not contain documentation that the child had received up-to-date immunizations per the age of the child. Evidence: 1. The record of Child #1did not contain a record of the immunizations the child had received. First date of attendance for Child #1 was 06/10/2019. 2. The owner of the center verified the immunization record had not been received. Plan of Correction: The parent of the child was called during the inspection and the center will obtain the immunization record as soon as possible.
