Standard #: 22VAC40-111-100-A Description: Based upon review of two children's files and provider interview, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. The record provider for child 1 indicated that child 1 began attending on 8/17/2020. The record did not include documentation of a physical examination.
2. The provider was unable to provide documentation of a physical examination for child 1.Plan of Correction: The provider responded with the following:
Documentation of physical examinations will be obtained prior to a child attending or within 30 days after a child has begun attending.
