Comments:
An unannounced monitoring inspection was conducted on May 22, 2024, from 9:45 am-11:20am. At the time of entrance, (107 ) children were in care with (18 ) staff members present. The inspector reviewed 5 children?s records and 5 staff records. Children were observed during snack time, engaged in group movement, manipulatives, making letter boxes and outdoor play time Medications were observed. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification of the Acknowledgement Form. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Violations:
Standard #: 22.1-289.035-B-4
Description: Repeat Violation
Based on a review of staff records, the center ahs not obtained a search of the child abuse and neglect registry for 1 staff that has resided out of the state in the past 5 years.
Evidence: Staff # 1, date of hire 1/5/2024 stated on their sworn statement that they had resided to the states of Delaware and New York in the past 5 years. There is no documentation that a search of the child abuse and neglect registry has been requested.

Plan of Correction: The out of state child abuse and neglect registry will be completed and the results emailed to the inspector within 10 business days.

Standard #: 8VAC20-780-60-A
Description: Based on a review of children records, 1 child's record did not have proof of birth within 7 days of enrollment.
Evidence: Child # 3, first day of enrollment 8/2021 does not have documentation of proof of birth.

Plan of Correction: The parent will provide the proof of birth and the documentation will maintained in the file.

Standard #: 8VAC20-780-510-I
Description: Based on a review of children medications and medication authorization documents, 1 child's medication does not have the prescription label in accordance with the prescribers instructions pertaining to dosage, frequency and manner of administration.
Evidence: Child #2 has a medication on site for a food allergy, the prescription medication does not have the prescription label with the prescribers instructions pertaining to dosage, frequency and manner of administration.

Plan of Correction: The parent will provide the prescription label for the child's medication.
