Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, October 15, 2019. The on-site inspection was held from 1:10 p.m. to 4:05 p.m. On this date, there were six children in the care of the provider (15 points). There were a total of seven children enrolled in the program. There were five children's records reviewed, along with the record for the provider. The provider and two children were interviewed. There were no medications being administered at the time of the visit, as is the policy of the provider. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. Snack consisted of homemade pumpkin cake that the children helped to make and milk. The children enjoyed learning about the letter E,e today. Some items reviewed/observed during the inspection included: first aid kit, emergency supplies, warm water temperature, refrigerator and freezer temperatures, required posted information, documentation of evacuation and shelter-in-place drills, choice time, hand washing and bathroom procedures, afternoon snack, diaper changing procedures, outdoor play and dismissal procedures.

Violations:
Standard #: 22VAC40-111-320-B
Description: Based on observation and confirmation with the provider, one electrical outlet did not contain a protective cover.

Evidence:
1. In the playroom, there was one electrical outlet on the wall with the shelving unit that did not contain a protective cover. It was accessible to the children.
2. The provider confirmed the outlet was missing a cover.

Plan of Correction: Immediately during the inspection, the provider replaced the outlet cover.
