**The inspection findings have been updated**

A monitoring inspection was initiated on 12/04/2023 and concluded on 12/04/2023. There were 16 children present, ranging in ages from 13 weeks to 5 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-140-A
Description: REPEAT VIOLATION

Based on a review of children?s records, the center failed to obtain documentation of a physical examination by or under the direction of a physician for each child before the child's attendance or within one month after attendance.

Evidence:
1. The record of Child 2 (DOE 10/11/2023) did not contain documentation of a physical in the record.
2. The record of Child 1 (DOE 10/17/2022) contained documentation of a physical dated 01/03/2023.

Plan of Correction: As of 12/7/2023 all children enrolled will not start until the physical and immunizations have been collected.

Standard #: 8VAC20-780-500-C
Description: Based on observation, the center failed to ensure toilet chairs were emptied promptly and cleaned and sanitized after each use.
Evidence:
1. Two toilet training chairs located in the Older Toddler and Two's Classroom
containing urine was observed at 11:38 am. All of the children and staff were outside on the playground at the time of observation.

Plan of Correction: Staff were retrained on emptying and sanitizing toilet chairs after each use.

Standard #: 8VAC20-780-520-A
Description: Based on a review of diaper ointments, the center failed to ensure that all over-the-counter skins products shall not be kept beyond the expiration date.

Evidence:
1. In the Older Toddler and Two's Classroom there was a tube of Desitin Diaper Cream that had expired in 03/23 and a tube of Vitamin A & D ointment that expired in 12/21.
2. Staff verified that Desitin Diaper Cream and Vitamin A & D had expired.

Plan of Correction: Staff were retrained to check diaper creams monthly and add the exp. date to the authorization form.
