Technical Assistance:
Training for staff regarding the proper handling of children, reporting of serious injuries

Comments:
A complaint inspection was initiated and concluded on 4/24/24 from 11:18 am to 1:00pm. The complaint was related to a child sustaining an injury while in care of staff and staff to children interactions. 3 staff members were interviewed, 1 child, 3 staff records and injury reports were reviewed.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names, list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Description: Based on staff interview and records review, the center did not inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurred while the child was under the supervision of the center and required outside medical attention.
Evidence:
1. Child # 1 ( 2 years old) sustained an injury while in care on 3/13/24. Staff # 1 stated that the child's injury required outside medical care.
2. The center did not report this injury to the department.

Complaint related: Yes
Description: Based on staff interview, staff did not always ensure care, protection and guidance of children while supervising them.
Evidence:
1. On 3/13/24, Child # 1 (2 years old) sustained an injury while in care of Staff # 2 in the 2's classroom.
2. Staff # 2 stated that close to nap time, she picked up Child # 1 who was laying on her cot by the hands to the point that Child # 1 was sitting on the cot.
3. Staff # 2 stated that after that, she lifted Child # 1 by her hands completely off the cot. Child # 1 started to cry.
4. As Staff # 2 lifted her, she hugged Child # 2 around the waist and placed on the diaper changing pad. Child # 2 continued to cry.
5. After Child # 1 was seen by the physician, it was determined that the child had suffered from Nursemaid's Elbow in her left arm.
