Comments:
An unannounced monitoring inspection was conducted on 5/3/2024 from 3:40pm-5:15pm. At the time of entrance, 43 children, ages 4 years to 11 years old were in care with 4 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during snack time, engaged in a staff led group game and discussion, and socializing with peers. Hand washing and table sanitizing were observed. Medication administration was not observed. There is no medication on site / being administered to children in care at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Repeat violations were found and are identified within this report.

Based on a review of records and interview, the licensee did not ensure that a written care plan is obtained for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
Five children's records were reviewed. The record for child #2 documented that this child has a tree nut, peanut and shellfish food allergy. The record for child #3 documented that this child has a dairy allergy. The record for child #5 documented that this child has an egg allergy. The records for child #2, #3, and #5 do not contain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Staff #1, who was in charge of the program during the inspection, confirmed that this documentation was not available for child #2, #3, and #5 during the inspection

Plan of Correction: Staff will speak with the parents of child #2, child #3 and child #5 this week and request that within 2 weeks, an allergy plan be obtained form their child's physician, or that the child's record be updated, as applicable, to indicate if the child is no longer allergic to the designated food allergens listed on the application and /or the Physical examination documents.
