An unannounced monitoring visit and facility tour was conducted on 6/8/2022. At the time of inspectors' arrival there were 15 infant to preschool aged children in care with 3 staff members and 1 administrator. Children were observed interacting with staff, snack, free play, diapering and hand washing. A sample of 5 children's records and 3 staff records were reviewed. The director reported no medications are being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 8VAC20-780-550-P Description: Based on record review and interview it was determined the center did not ensure that the written record of children's injuries contained all of the required information.
1. There were multiple injury reports that did not contain future action to prevent recurrence and/or the date, time, and method of parental notification.
2. The director confirmed that the injury reports were missing some of the required components.Plan of Correction: Director will retrain staff on completing injury reports.
