Comments:
An unannounced on-site monitoring inspection was conducted on 4/25/24 as part of the Provisional licensure period. The inspection was conducted from 3:20 om to 3:55 pm. The Inspector reviewed compliance in the areas listed above. There were 8 children present with 2 caregivers for a total of 21 points. The inspection included a review of one child and one caregiver record. The inspection also included a tour of the home, caregiver interviews, observations and documentation review. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-680-A
Description: Caregivers hands shall be washed with soap and water after diapering.
A caregiver was observed failing to wash their hand between diapering 3 children.

Plan of Correction: I will retrain the staff to wash hands after each diaper change regardless if they wear gloves or not.
