An unannounced annual Subsidy Health and Safety Inspection (SHSI) was conducted on 12/20/2023 from 12:40pm until 1:40pm. At the time of the inspection there was one child in care and one household child for a total of 4 points. Emergency supplies, evacuation drills, and records for the caregiver, children and household members were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Standard #: 8VAC20-790-190-B Description: Based on record review and interview, it was determined the vendor did not ensure that all required information was maintained for each child enrolled.
Evidence:
1.The record for child #1 did not contain documentation of immunizations, emergency contacts and written authorization for emergency medical care.
