An unannounced renewal inspection was conducted on 07/11/2023 from 9:30 a.m. to 12:20pm. There were 62 children ages two months to age 12 years old supervised by 11 staff. The physical plant, outdoor playground area, programming, attendance, menus, five staff records, five children?s records, emergency drills, injury reports, emergency supplies, and policies were reviewed. No medication was on site today. Children were observed participating in group play, circle time and outside play. Diapering and handwashing procedures were also observed. There were two staff present with current certification in Medication Administration Training (MAT). All staff records reviewed had CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-160-A Description: Based on a review of five staff records, the center did not obtain documentation of a tuberculosis (TB) screening for staff at the time of employment or prior to coming in contact with children.
Evidence: The record for Staff #1, with a start date of 1/9/23, and Staff #3, with a start date of 4/11/23, did not contain documentation of a tuberculosis screening at the time of inspection.Plan of Correction: T.B. tests were completed by staff.
Standard #: 8VAC20-780-70 Description: Based on a review of five staff records, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment
Evidence: The record for Staff #3, with a start date of 4/11/23, and Staff #5, with a start date of 6/21/23, did not contain documentation of two references at the time of inspection.Plan of Correction: Staff files will be updated with references.
Standard #: 8VAC20-780-240-B Description: Based on review of five staff records and interview with administrator, the center did not ensure that staff completed orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities.
Evidence: The record for Staff #1, with a start date of 1/9/23, and Staff #4, with a start date of 12/28/22, did not have documentation of orientation training at the time of inspection. The director confirmed that orientation was not complete.Plan of Correction: Orientation was held on 7/11/23. Going forward, all orientations will be held on/before new staff start work. Staff #4 is no longer employed at center
Standard #: 8VAC20-780-270-A Description: Based on observation, not all areas of the center were maintained in a clean, safe, and operable condition.
Evidence:
1. On the school age playground a metal swing set structure had been removed and placed in a corner of the playground on its side. The metal footings of the structure and eight rusted screw threads, five threads in length, were exposed and create an impalement and scratch hazard.
2. On the school age playground wooden boards were on the ground next to the wooden fence, one of the wooden board contained four rusted nails, with one nail protruding upward causing a impalement and scratch hazard.
3. On the toddler playground there were 10 foam building blocks that had teeth marks with chunks of the foam missing from the blocks.
4. In the toddler and two?s class, the paint on the window sills were chipped.
5. In the toddler room the plastic blinds covering the windows were broken creating sharp edges posing a scratch hazard.Plan of Correction: Swing has been removed from School Age playground.
Boards have been removed from School Age playground.
Foam blocks have been removed from Toddler playground. Window sills will be sanded and painted.
Blinds will be replaced, until replaced blinds are moved to a higher adjustment.
Standard #: 8VAC20-780-330-B Description: Based on observation, the center did not ensure that where playground equipment was provided, resilient surfacing was maintained in fall zones.
Evidence: There was less than one inch of mulch observed at the base of three slides and four swings on the playground. The mulch has deteriorated into dirt and grass is growing throughout the fall zones. Six inches of resilient surfacing is required based on the height of the play equipment and the type of resilient surfacing.Plan of Correction: All areas have been provided with mulch to protect fall zones.
Standard #: 8VAC20-780-340-F Description: Based on observation and interview the center did not ensure that children under 10 years of age, were within actual sight and sound supervision of staff at all times.
Evidence: During the inspection two four-year old children were observed walking from the Pre-K class into the empty three-year-old class to use the restroom. The Pre-K teacher was not able to see or hear the children in the restroom. The teacher confirmed that the Pre-K children go into the three year class to use the bathroom unattended when the three year class is on the playground.Plan of Correction: Training in regards to sight and sound will be held for all staff.
Staff was reminded to always assure students are within sight and sound at all times.
Standard #: 8VAC20-780-500-B Description: 8VAC20-780-500-B Based on observation, not all diapering areas were provided a leakproof covered receptacle for soiled linens and a foot-operated disposal system.
Evidence:
1. In the two-year-old class, there was no liner in the white laundry basket for soiled linens.
2. In the two-year-old class, the lid on the trash can used for diaper disposal was broken, and could not be operated by foot.Plan of Correction: Mesh garment bag was placed in hamper.
Trash can was purchased for classroom.
