An unannounced monitoring inspection was conducted on 7/27/23 from 11:35 a.m. to 1:00 p.m. with the Director. There were 70 children in care, ranging in age from three months to12-years-old, supervised by 11 staff. The children were observed eating lunch and preparing for nap. Lunch was hot dogs, tater tots, applesauce, and milk. Required postings were observed. The physical plant, outdoor playground areas, programming, policies, attendance, and menus were reviewed. Please complete the "Plan of Correction" and "Date to be corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 8/15/23. Standard #: 8VAC20-780-70 Description: Repeat
Based on a review of four staff files, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence: The record for Staff # 1, with a start date of 4/11/23, and Staff #2, with a start date of 6/21/23, did not contain documentation of two references at the time of inspection.Plan of Correction: Both staff have updated files and have references.
Standard #: 8VAC20-780-80-A Description: Repeat
Based on a review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurs
Evidence: In the three year class there were 13 children present in the classroom, the written record reflected there were 11.Plan of Correction: Measures were taken to ensure that attendance is taken properly. Staff has been trained and spoken to about this.

Based on observation, not all areas of the center were maintained in a clean, safe, and operable condition.
Evidence:
1. On the school age playground wooden boards were on the ground next to the wooden fence, two of the wooden boards contained four rusted nails, with one nail protruding upward, a separate nail tip sticking out of the wood, and a metal stake with a pointed edge, approximately 12 inches in length, was discovered in the grass creating an impalement and scratch hazard.
2. On the school age playground, a metal stake with a pointed edge, approximately 12 inches in length, was discovered in the grass.
3. On the toddler playground there were 10 foam building blocks that had teeth marks with chunks of the foam missing from the blocks.Plan of Correction: 1. Wooden boards were picked up and removed.
2. Metal stake was picked up and removed.
3. All foam blocks were picked up and thrown away
Standard #: 8VAC20-780-330-B Description: Repeat
Based on observation, the center did not ensure that where playground equipment was provided, resilient surfacing was maintained in fall zones and that the required fall zone in front of slide exits maintained a six foot fall zone.
Evidence:
1. There was less than one inch of mulch observed at the base of three slides and fall zones of the composite play structure located on the three and four-year old playground. The mulch has deteriorated into dirt and grass is growing throughout the fall zones. Six inches of resilient surfacing is required based on the height of the play equipment and the type of resilient surfacing.
2. A plastic wading pool was propped against the building next to the exit of one of the slides on the composite play structure. The fall zone measurement from the slide exit to the propped wading pool measured four feet, three inches. When the wading pool was removed, the fall zone measurement from the slide exit to the building was five feet, six inches. The requirement is a six foot fall zone in front of slide exits.
3. There was less than one inch of mulch observed in the fall zones of the four swings, approximately three inches of mulch were directly under each of the swings.Plan of Correction: 1. Mulch was measured and replaced
2. Fall zones has been measured to be six feet
3. Mulch was measured and added.

Based on observation, not all diapering areas were provided a leak-proof covered receptacle for soiled linens.
Evidence:
1. In the infant class, a mesh garment bag was used as a liner for the white laundry basket used for soiled linens.
2. In the two-year-old class, there was no liner in the white laundry basket for soiled linens.Plan of Correction: All baskets have been provided a leak proof bag.
Standard #: 8VAC20-780-560-F-4 Description: Based on observation and interview, the center did not ensure that children, three years of age and younger, are served foods that are considered potential choking hazards.
Evidence:
1. At the time of inspection, hot dogs were observed in the two and three-year classrooms, the hot dogs were cut in half and not cut in quarters to prevent choking. Staff confirmed that hot dogs were not cut into smaller pieces before serving them to the children.
2. In the toddler class, an interview with staff revealed that hot dogs, cut vertically and horizontally were served to children sixteen to 24 months old.Plan of Correction: 1. If served, hot dog will be cut into smaller pieces to avoid choking hazard.
2. Hot dogs will avoid being served.
Standard #: 8VAC20-780-560-G Description: Based on observation, the center did not ensure that when food is brought from home that it is clearly dated and labeled in a way that identifies the owner.
Evidence: In the infant classroom, a sippy cup, containing pink liquid, was observed in the refrigerator with no name or date.Plan of Correction: Staff has been spoken to and all food brought from home has been labeled.
