Comments:
An unannounced focused inspection was conducted 10/18/2023 from 11:00am through 12:35pm. There were 47 children present with 9 staff and all classrooms were in ratio. The physical plant, outdoor playground area, nutrition, 1 staff record, attendance, and diapering procedures were inspected. Children were observed making bracelets, creating pumpkin crafts, eating lunch, handwashing, and participating in center play. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-80-A
Description: REPEAT VIOLATION

Based on review of the written attendance, the center did not maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.

Evidence: In the Toddler room, there were 7 children signed in on the written attendance, however, there were only 6 children present at the time of inspection.

Plan of Correction: Attendance was corrected on the spot. Attendance has been moved to Brightwheel.

Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATION

Based on observation, the center did not ensure that areas inside the center were maintained in a safe condition.

Evidence: In the Three's classroom, there were two small metal hooks mounted on the wall positioned approximately three feet from the ground that could snag skin or cause a more serious injury if a child were to run into the hooks located at their eye level.

Plan of Correction: Remove hooks

Description: REPEAT VIOLATION

Based on observation, the center did not ensure that the required amount of resilient surfacing was underneath and surrounding playground equipment in order to create a sufficient fall zone of six feet.

Evidence:
1. On the Toddler playground, the yellow spinning riding toy was within four feet three inches of the wooden mulch barrier and within four feet six inches of the climber equipment next to it.
2. Also on the Toddler playground, the slide attached to the climber equipment measured four feet six inches from the slide exit to the wooden mulch barrier.

Plan of Correction: Riding toy will be removed to allow proper space for the slide.

Description: REPEAT VIOLATION

Based on observation, the center did not ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hands nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence: At the time of inspection, the trash can located in the diapering area of the Toddler classroom required the use of staff's hands to lift the top off the trash can to dispose of soiled diapers, as the foot-operated mechanism was not functioning. Soiled diapers were observed in the trash can.

Plan of Correction: Trash can was fixed on the same day. Trash cans will be checked to ensure proper function daily.

Standard #: 8VAC20-780-560-G
Description: REPEAT VIOLATION

Based on observation and staff interview, the center did not ensure that when food is brought from home that the food container is clearly dated and labeled in a way that identifies the owner and that unused portions of opened food shall be discarded by the end of the day or returned to the parent.

Evidence: In the infant room, a fruit and veggie blend was observed on the counter and was not dated or labeled to identify the owner. In addition, staff advised the fruit and veggie blend item was from yesterday that the child did not finish.

Plan of Correction: Juice bottle was thrown away on the spot. Staff retrained on proper labeling.
