Comments:
An announced nonmandated inspection was initiated on 12/11/2023 and completed on 12/11/2023. A review of staff personnel files was conducted by inspectors, Margaret Allworth, Mahrukh Aziz, Leah Pagala, Alyssa Simons and Derek Acosta at the Pennino SACC office. Record reviews new hires are documented with the SACC at Hunt Valley file.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A
Description: Based on record review, the organization did not ensure that all employees undergo a background check every five years from the date of the initial check.

Evidence: Staff #4 criminal record check was originally conducted June 13 2018 and a repeat check was not documented to have a occurred.

Standard #: 8VAC20-780-160-C
Description: Based on record review, the organization did not ensure that, at least every two years from the date of the first initial screening or testing, staff members obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
Staff #3 and Staff #4 Records did not contain documentation of follow-up screenings.

Staff #4 date of previous screening was listed as November 8, 2021
