Comments:
An unannounced, on-site complaint investigation began on 4/17/2024 in response to an allegation received on 4/11/2024 that the family day home was operating over the licensed capacity. Follow-up inspections occurred on 4/18/2024 and 4/29/2024. 9 children?s records were reviewed. The inspections included document review, tour of the home, interviews, and observations. Additional documents were received by partner agencies.

Based on available evidence gathered during the investigation, the complaint is valid.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/23/2024. A POC submitted after this date will not appear on the public website.

Description: Providers must ensure that no person shall supervise children prior to the facility obtaining a completed fingerprint background check. Caregiver 2 did not have a completed fingerprint background check. Caregiver 2 provided care to the children beginning March 18, 2024.

Plan of Correction: Moving forward I will make sure all people, employees & caregivers have a fingerprint background check & a central registry check before supervising children in care.

Description: Poisonous materials shall be stored in an area inaccessible to children. On 4/17/2024, cleaning chemicals were on the staircase and the ledge at the staircase that children use to enter the home.

Plan of Correction: All cleaning supplies will be put away as soon as they enter the home, in a child proof cabinet.
