Technical Assistance:
Please review the American Academy of Pediatrics guidance not to use sitting devices, including car safety seats, strollers, swings, infant carriers and infant slings for routine sleep. Infants who are younger than 4 months are especially at risk because they might assume positions that can create risk of suffocation or airway obstruction. Infants who fall asleep in a sitting device should be moved to a crib or other safe sleep surface.

Comments:
A renewal inspection was conducted on 05/07/2024 from approximately 9:15 p.m. to 11:50 a.m. There were eleven children present ranging from infants to three years old and three staff supervising. Five child records and five staff record were reviewed. The information gathered during the inspection determined violations with applicable standards or law.

Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.

Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on record review, the provider failed to ensure that staff shall complete the Virginia
Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
The record for S3, employed 11/20/2023 did not contained a certificate of completion of the Virginia Preservice Training.
The record for S1, employed 12/18/2023 did not contained a certificate of completion of the Virginia Preservice Training.

Description: (Repeat violation)

Based on observation, the provider did not ensure that all areas of the center be
maintained in a clean, safe, and operable condition.
Evidence:
The refrigerator used to store children?s lunch boxes was caked with unidentified debris.
The bathroom used by children had broken floor tiles creating a fall hazard.
The plastic henge cover on the children?s bathroom door is ripped exposing the door henges.
The diaper changing pad in the infant room was ripped and exposing absorbent foam material.

Description: Based on observation, the staff-to-child ration of 1:4 for children birth up to 16 months was not followed when children were in care.

Evidence:
Licensing inspector entered the infant room and observed five infants with S4. S4 reported a staff member calling out that morning.

Description: Based on observation, the staff-to-child ration of 1:5 for children 16 months up to 24 was not followed when children were in care.

Evidence:
Licensing inspector entered the preschool class and observed six children with S1. The youngest child in the classroom was not yet 2 years old requiring a 1:5 ratio.
