- Technical Assistance:

Consultation was given on using model forms for children's records, liability insurance declaration, provision for emergency preparation, physical exam required within 30 days, Information for Parents, and any walking field trip permission forms. All forms can be found on the VA DOE Licensing website at childcareva.co.
Written emergency plan will need to be updated to the model form vs the County permit form.
SCC documentation will need to be filed to add "Little Brains Home Daycare" to the License. Provider wants that on the license so Inspector will not issue until the SCC documentation is completed. Form given to provider to complete the fictitious name request.

An announced initial study was conducted. Two children were present with the provider for 6 points which is within ratios for supervision. Observed the home/physical space, emergency preparation, trainings, background checks, nutrition and record keeping. The provider's 2 cribs meet current CPSC requirements. Dedicated space is in the basement for the daycare and the entrance is from the back through the lower level. Outside play area as well on the patio. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued. Feel free to contact me if you need any assistance.
