Technical Assistance:
Technical Assistance was provided to the provider regarding:
-menu/meal components
-a crib must be obtained before a license can be issued to care for children under 13 months. Each child must have their own crib as well.

Comments:
An announced initial inspection was conducted on 3/20/2024 from 8:40am to 10:15am with the provider. The provider is planning to operate Monday through Friday from 6:00am to 6:00pm. The provider is planning to care for up to 11 children from 13-months to 12-years-old. There were five children in care, three daycare children and two household children, ranging in age from 13-months to three-years-old, for a total of 13 points. Children were observed finishing breakfast, washing hands, playing with blocks, cleaning up, doing circle time, and playing with playdough. Three child records were reviewed. The provider and household member records were reviewed prior to the on-site inspection. The provider has current certification in CPR and First Aid. The provider does not intend to administer medication at this time, but is planning to obtain Medication Administration Training (MAT) in order to administer medication in the future. A walk-through of the home was conducted to check for hazards. The home is equipped with a variety of age-appropriate supplies and materials. Meals are provided by the provider and menus were reviewed. Policies and procedures were reviewed with the provider. The first aid kit, flashlight, weather-band radio, carbon monoxide detector, and smoke detectors were observed. Emergency numbers, evacuation maps/procedures and daily schedules were posted. The attendance was also reviewed. A conditional license will be recommended. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not observed at the initial inspection. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 4/5/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-800-60-A
Description: Based on review of three child records, the provider did not ensure that parents signed acknowledgement of the receipt of required policies and procedures. Evidence: The records for Child A, Child B, and Child C did not contain parent signatures for the required policies.

Plan of Correction: I will update policy handbook and have parents resign an updated parent disclosure signed by each parent.

Standard #: 8VAC20-800-430-B
Description: Based on observation, the provider did not ensure that pressure mounted gates were not used as protective barriers at stair openings. Evidence: On 3/20/24, a pressure mounted gate was observed at the bottom of the stairs in the daycare area.

Plan of Correction: An appointment has been made with a handyman to securely drill the gate on the stair opening.
