Comments:
An unannounced inspection was conducted on 5/17/2024 in response to a complaint that was received by the licensing department on 05/15/2024 regarding allegations in the areas of supervision and injury reporting. On 05/17/2024, the inspector was on site from approximately 8:40 am to 10:12 am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Three child records were reviewed on site.

The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on a review of documentation and interview, the center did not ensure that a parent was notified by the end of the day of a minor injury as required.

Evidence:
1. The record of child #1 (date of attendance: 2/23/2024) contained a minor injury report dated 5/10/2024. Staff interviewed stated that they did not notify the parents of child #1.
2. Administration acknowledged that the parents were not notified within the required time frame.

Plan of Correction: The center-wide policies and procedures for injury reporting have been reformulated to include immediate electronic notification via Kaymbu app to ensure consistent timely notification of all injuries to parents. Lead teachers initiating, admin ensuring compliance.

Description: Based on a review of documentation and interview, the center did not ensure to maintain a written record of children's minor injuries that included the required information.

Evidence:
1. Thirty-eight (38) injury reports were reviewed. Of the 38, five were incomplete and missing the future action to prevent recurrence of the injury. Injury reports were required to contain the 1. Date and time of injury; 2. Name of injured child; 3. Type and circumstance of the injury; 4. Staff present and treatment; 5. Date and time when parents were notified; 6. Any future action to prevent recurrence of the injury; 7. Staff and parent signatures or two staff signatures; and 8. Documentation on how parent was notified.
2. Administration acknowledged that the injury reports were incomplete.

Plan of Correction: 1. Old incomplete injury report forms missing the preventative action line have been removed from the rotation. Staff have been retrained on requirements for injury reporting to ensure completion moving forward.
