An unannounced complaint inspection was conducted on 1/30/2020 from 8:15 a.m. to 10:45 a.m. in response to similar complaints that were received by the licensing office on 1/10/2020 and 1/29/2020 regarding alleged noncompliance in the areas of supervision / staff to child ratios. Concerns were also raised regarding cleaning and sanitizing procedures as well as communication with parents regarding illnesses in the center.
The information gathered during the investigation supports the allegation of physical plant maintenance of the outdoor play equipment. This portion of the complaint was Valid and the violation appears on the violation notice.
The information gathered during the investigation does not support the allegations regarding to staff to child ratios, supervision, emergency preparedness and discipline so this portion of the complaint is found to be Not Valid.

Standard #: 63.2(17)-1716-A-3-c Description: Based on observation and interview, the religious exempt child day center did not always employ supervisory personnel in accordance with the required ratio of staff to children.
Evidence:
On Jan 30, 2020 at approximately 8:20 a.m., the inspector observed 11 children in the two year old classroom with one staff. The staff present in the classroom (staff #1) stated that another staff was scheduled to be in shortly.
Approximately 11 minutes later, two more children had arrived bringing the ratio to 13 children and 1 staff. A second staff entered the classroomat about 8:35 bringing the ratio of adults to children to 2:13.Plan of Correction: The director will review with all classroom staff the necessity of utilizing the two way radios to communicate with the staff in charge letting them know that they are at maximum attendance for the required ratio. This will allow the site supervisor to assist and will allow staff in other rooms to assist as they are able to and/or shift children as needed to maintain the proper staff to child ratios in all groups.
The Program Director will continue to review staffing and attendance to ensure that proper ratios can be maintained throughout the center.
