Comments:
An unannounced monitoring inspection was conducted on 4/26/2024 from 10:00am to 12:53pm. There were 192 children (2-5yrs) directly supervised by 40 staff. The physical plant, 9 staff records, 10 children?s records, 7 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, preparing for and eating lunch. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: reviewed 10 children's records and found that:
1) Child 5 did not have documentation of authorized pick-ups
2) Child 3 did not have documentation of addresses for either emergency contact
3) Child 5 did not have documentation of emergency contacts
4) Child 2 had documented a parent as an emergency contact

Description: Based on observation, the facility did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed a spray bottle with soap water was accessible to children on sink counter

Description: Based on observation, the facility did not ensure that when food is brough from home, the following shall apply: the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.
