1. Child enrollment records are to include documentation of their proof of identity.
2. Inspector provided information about out-of-state background checks.
3. Inspector will send local zoning information.
- Comments:

Conducted an unannounced renewal inspection at 12:18pm. Observed 22 children + 3 direct-care staff in the extended day afterschool program. Ratios and supervision were in compliance. Children were observed having active outdoor play, playing table games, washing hands, eating snack and departing for the day. The program areas were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Standard #: 22.1-289-036-B-4 Description: Based on records reviewed, 2 of 5 persons listed on the renewal application as an agent of the school do not have required background checks.
Evidence:
1. Person/Agent #2 does not have a completed criminal record check.
2. Person/Agent #3 does not have a completed central registry abuse and neglect search.Plan of Correction: Person #2 has scheduled a fingerprint appointment to be completed 5/14/24.
Person #3 submitted abuse + neglect search as of 5/9/24.
Standard #: 22.1-289.035-B-4 Description: Based on records reviewed, 1 of 4 staff records did not include a required out-of-state background check within the first 30-days of employment. Evidence: The record for Staff #7 did not include a completed out-of-state central registry abuse and neglect search. Staff #7 began working in October 2023. Plan of Correction: Staff #7 has submitted out of state checks as of 5/9/24.
