Technical Assistance:
Had extensive conversations with center director regarding staff behavioral guidance and supervision, hand washing techniques and the food menu. We also reviewed recent changes to background check requirements.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 8:40am and 1:30pm. At the start of the inspection there were a total of 64 children with 11 staff; all ratios were incompliance. A total of 6 children?s files and 5 staff files were reviewed. The children were observed being dropped off by their parents, eating breakfast, participating in circle time and eating lunch. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain updated immunizations every six months for children under the age of two. Evidence: Child #3 is currently 22 months old and their most recent immunizations are from then they were 7 months old. Child #4 is currently 15 months old and their most recent immunizations are from when they were 2 months old. Child #5 is currently 18 months old and their most recent immunizations are from when they are 4 months old.

Plan of Correction: I've been going through the children's files and letting them know if we're missing information. I'll request these updated forms tonight or tomorrow.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain a physical exam for one child within the first 30 days of attendance. Evidence: Child #4's first date of attendance was 7/10/2017. There is no physical exam on record.

Plan of Correction: I've been going through the children's files and letting them know if we're missing information. I'll request these updated forms tonight or tomorrow.

Standard #: 22VAC40-185-160-A
Description: Based on record review, one staff member did not submit documentation of a negative tuberculosis (TB) screening within the first 21 days of employment. Evidence: Staff #2 date of hire was 3/13/2018 and there is no documentation of a negative screening on record.

Plan of Correction: I'll look at the TB results more clearly for the results.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information. Evidence: Child #2 file did not contain a street address for their second emergency contact. Child #5 file did not contain street addresses for both emergency contacts.

Plan of Correction: I've been going through the children's files and letting them know if we're missing information. I'll email these parents parents to get these addresses.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place that prevents access by children. Evidence: There is a bleach cleaner stored in the sliding cabinet under the diaper changing table in the Infant B classroom. There is no lock on the cabinet.

Plan of Correction: I'll have the teachers move the cleaners to a different cabinet.

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning materials were located on the same shelf as food and single-serve articles. Evidence: There is an air freshener and bleach cleaner stored with infant rice cereal and serving napkins in a sliding shelf in the Infant A classroom.

Plan of Correction: I'll have the teachers move the cleaning materials.

Standard #: 22VAC40-185-500-A
Description: Based on observation, children's hands were not washed prior to eating. Evidence: 1. During the inspection, a child was dropped off by their parents in the Preschool classroom. Approximately 5 minutes after arrival the child was served breakfast. The child was not directed to wash their hands prior to eating. 2. The children in the Infant B classroom were observed being put into their feeding chairs to eat breakfast. The children did not have their hands washed prior to the teacher starting to serve the children.

Plan of Correction: We do have hand washing instructions in all of the classrooms, and I'll remind all of the teachers of the importance of handwashing.

Standard #: 22VAC40-185-520-A
Description: Based on observation, over-the-counter skin products were kept beyond their expiration date. Evidence: Child #6 had a diaper cream in the classroom that had expired 6/2015.

Plan of Correction: I'll let the teachers know that they need to check the expiration dates on their creams and moving forward teachers will need to check when parents hand them creams.
