A Subsidy Health and Safety inspection was conducted on 05/14/18 to review supplemental health and safety requirements. There were 11 children in direct care and supervision of 8 staff members. The children were observed watching a movie, obtaining therapy, doing a craft activity and playing a game. A licensing renewal inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11 Description: Based on record review, a written allergy action plan was not obtained for children with diagnosed food allergies. Evidence: Child # 1's record indicated that she is allergic to fish and seafood. Child # 1's record did not have a written allergy action plan. Plan of Correction: I will get the plan from the parents.
