The licensing inspector conducted an unannounced renewal inspection Tuesday, May 22, 2018 from approximately 10:00 AM to 2:30 PM. The following ratios were observed with a total of 54 children present during the inspection: Early Head Start (1-2 years) - 8 children with 2 staff Early Head Start (2 years) - 8 children with 2 staff Head Start (Threes & Fours) - 15 children with 2 staff Preschool I (Infants ? 2 years) - 11 children with 3 staff Preschool II (Threes & Fours) - 12 children with 2 staff The children and staff were engaged in a variety of activities when the licensing inspector arrived. Children were observed being held and fed, looking at books, playing in the gym, playing outside, taking bathroom breaks and washing their hands and eating lunch. Lunch consisted of beans and rice salad with cheese, corn, pineapples and milk. Later children were observed resting at naptime. All areas of the center including classrooms, hallways, kitchen, bathrooms and playgrounds were inspected. Measurements of resilient surfacing material (mulch) were not taken due to moisture levels. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, daily sheets, Infant feeding schedules, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The liability insurance policy is current through 02/09/19. The center?s first aid kit and emergency supplies were inspected. Medication is administered and medication and authorization forms were reviewed. Transportation is provided and the vehicle and transportation documents were reviewed. Last documented health inspection: 04/03/18 Last documented fire inspection: 12/19/17 During the inspection, five children's records and five staff records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. The renewal application was received prior to the expiration of the current license. Standard #: 22VAC40-185-440-B Description: Based on observations, the center did not ensure the cribs were identified for use by a specific child. Evidence: During the inspection, the licensing inspector asked the staff in the Preschool I classroom if the three cribs were used by children. The staff in Preschool Classroom stated three children use the cribs. The licensing inspector observed one crib labeled with a child's name. The licensing inspector asked the staff if the other two cribs were labeled or if there was a crib chart and staff stated the cribs were not labeled and they did not use a crib chart. Plan of Correction: Corrected during the inspection.
