Technical assistance was provided in the following areas of the standards: Background checks; 22VAC40-185-(4)-270-A (Building maintenance)

Comments:
An unannounced monitoring inspection was conducted on 5/21/18 from 9:45am - 1:00pm. During the inspection there were 63 children ages five months old through five years old in care with 14 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch and during rest period. Records were reviewed for six children and six staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. On the large playground, there was two inch deep of standing water on the nine foot by nine foot concrete pad by the basketball hoop. 2. The fence at the rear of the playground has been knocked over and is missing the two of the top posts. 3. The vents in the ceiling in the Pre-K room are dirty and need to be cleaned. 4. Staff #2 (Program director) confirmed the items listed above were currently not in a clean, safe and operable condition.

Plan of Correction: The facility responded: The maintenance man was contacted during the inspection, and showed up to work on removing the water from the concrete pad and cleaning the vents. The fence on the playground was knocked down in a storm and is scheduled to be repaired this week.

Standard #: 22VAC40-185-440-J
Description: Based on observation it was determined that the facility did not ensure that when cribs are occupied there should be 30 inches of space on the service side of the crib for staff to gain access to the crib. Evidence: 1. There were two occupied cribs in the Infant room that had less than 30 inches of space on the service side of the crib. 2. Staff #2 (Program Director) confirmed that there were two cribs that did not have 30 inches of space on the service side of the cribs.

Plan of Correction: The facility responded: Corrected during the inspection. Both cribs were moved so that there was 30 inches of space on the service side of the crib. Staff will be re-trained to ensure there is always 30 inches of space on the service side of occupied cribs.

Standard #: 22VAC40-185-550-M
Description: Based on a review of 21 injury reports and interviews, it was determined that the facility did not ensure that the written record of children's serious and minor injuries includes all of the required items. Evidence: 1. Four of the twenty-one injury reports reviewed did not include the method of how the parent was notified and the time the parent was notified. 2. Staff #1 (Assistant Director) reviewed the injury reports and confirmed there were four injury reports that did not include all of the required information.

Plan of Correction: The facility responded: Staff will be re-trained to ensure all injury reports are completed correctly to include all of the required items. Center management will review all injury reports to ensure they contain all of the required information.
