Comments:
An unannounced visit was conducted to investigate a self-reported incident that occurred on 04/18/2018 regarding behavioral guidance. The investigation was conducted on 04/19/2018, from 11:00am - 3:30pm, and a follow up was conducted on 04/26/2018. Based on the evidence gathered from staff interviews, and records reviewed, it was determined that the preponderance of evidence does support a violation, which can be found on the Violation Notice. Violations:
Standard #: 22VAC40-185-400-A
Description: Based on staff interviews, it was determined that a staff member did not use behavioral guidance to promote a child's well-being and growth, by interacting with the child to provide needed help, comfort and support. Evidence: Staff#1 stated that they used a harsh tone with a two year old child, after the child had a toileting accident. The staff confirmed that the two's class was on the playground, when the staff member took child#1 to the restroom. The child tried to use the potty for approximately 10 minutes, then the staff member and the child began to walk back to the playground. The staff member stated that they noticed child#1 trying to hide behind the diaper changing table, and shortly after the child began to cry. On their way back to the playground the child had a toileting accident. The staff member admitted that although they did not yell at the child, they did use a harsher tone than they should have when they realized that the child had a toileting accident.

Plan of Correction: All staff received training on positive behavioral guidance and strategies.
