Comments:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, May 24, 2018 from approximately 1:20 pm to 4:40 pm to determine the center's compliance with licensing standards. During this inspection, the Director/ Corporate Manager was present and assisted the licensing inspector throughout this inspection. The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection was 31 children. The infants were observed by the licensing inspector engaged in free play in their designated area on the carpet engaging with staff and resting. The toddler and preschool children were observed napping. The children's lunch today consisted of turkey and cheese sandwich, baked beans, fruit salad and milk. During this inspection,the licensing inspector engaged and interacted with staff and children when appropriate. During this inspection, the school-aged children were not present. The required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, emergency supplies, fire and shelter-in-place drills log, liability insurance policy, and emergency preparedness plan. The classrooms, indoor and outdoor play areas, and bathrooms were inspected for compliance. There were five children?s records reviewed, five staff records reviewed and one medication reviewed.
The center's first aid kit and emergency supplies, and playground areas were inspected.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of five children's records reveiwed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: before the child's attendance or within one month after. Evidence: During the inspection conducted on May 24, 2018, there was no documentation of a physical examination in the record for Child #2 (start date 4/16/18).

Plan of Correction: We will have the parent to provide a copy of the physical.

Standard #: 22VAC40-185-160-A
Description: Based on record review, in one of five staff records reviewed, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening no later than 21 days after employment. Evidence: During the inspection conducted on May 24, 2018, there was no documentation of a TB screening in the record for Staff #1 (start date 4/23/18).

Plan of Correction: We will have the staff member to obtain a TB screening or test.

Standard #: 22VAC40-185-160-C
Description: Based on record review, in one of five staff records reviewed, the licensee did not ensure that staff resubmit TB test results every two years. Evidence: During the inspection conducted on May 24, 2018, the TB test results for Staff #5 were more than two years old and were dated 5/12/16.

Plan of Correction: We will have the staff member to obtain a TB screening or test.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in three of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information.

Evidence: During the inspection conducted on May 24, 2018, the following information had not been documented in each child's record:
Child #4 and Child #5-the names of any additional programs or schools that the child is currently attending and the grade or class level if applicable.

Child #3-the work phone number for one parent and the names of any additional programs or schools that the child is currently attending and the grade or class level if applicable.

Plan of Correction: The Director will have the parents fill out the information upon pick up.

Standard #: 22VAC40-185-550-D
Description: Based on review, the licensee did not ensure that center implemented a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on May 24, 2018, there was documentation of one of two required shelter-in-place drills practiced in 2017.

Plan of Correction: Two will be done this year.
