Comments:
Today an unannounced monitoring inspection was conducted between the hours of 1:00pm and 4:30pm. I observed 64 children directly supervised by 10 staff. Ratios and supervision were in compliance. Children were observed arriving to the program, doing homework, playing on the playground, reading, and participating in group play. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 5 children?s records, 5 staff records, medications, injury reports, emergency supplies, and policies were inspected. The school areas used for extended day care were found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Emergency preparedness supplies in order. Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain complete children's records. Evidence: Child#1, #2 and #3 were missing addresses for 2 of 2 required emergency contacts.

Plan of Correction: We are updating the application process to include the information.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the center does not have documentation to verify that a staff member received orientation training by their first day of assuming job responsibilities. Evidence: Staff#1was hired on 06/01/2017, the staff members' file does not contain orientation documentation.

Plan of Correction: We will have the staff member sign the checklist.

Standard #: 22VAC40-185-510-C
Description: Based on review of children's medications, it was determined that one medication was on site with expired authorization forms. Evidence: Child#3 has a long term emergency prescription medication on site with an expired medication authorization form. The authorization form expired on 05/05/2018.

Plan of Correction: We will revise the form to include the information needed.
