Technical Assistance:
Inspector discussed the updated background check regulations with the Director during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 10:00am-12:29pm. At the time of inspectors' arrival there were 68 preschool aged children in care with 8 staff members and 1 administrator. Children were observed participating in arrival, indoor and outdoor play, toileting and handwashing, and transitioning between activities. A sample of 6 children's records and 6 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are noted on the Violation Notice and were discussed with the director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In the Red, Green, Yellow, Orange and School Aged rooms there was chipped/broken drywall and peeling paint. 2. In the Red room there was dust buildup along the bottom shelves of the bookcases/cubbies. 3. In the Bathroom adjoining the Red and Green rooms there was broken and peeling drywall/plaster behind the three toilets. 4. In the Green room there was a build up of grime on the walls as well as dust buildup along the bottom shelves of the bookcases/cubbies. 5. In the Yellow room there was a buildup of dust/grime around the vent on the ceiling. 6. In the Commons there was a hole in the wall by the round window closest to the stage.

Plan of Correction: Some of the cleaning/dusting needs were taken care of during this inspection by the staff. The Organization is currently recruiting maintenance personnel and will complete the other repairs as soon as possible.

Standard #: 22VAC40-185-330-B
Description: Based on observation it was determined that the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: There were weeds growing in the mulch in the fall zones of the play structures.

Plan of Correction: The Organization is currently recruiting maintenance personnel and will complete the grounds maintenance as soon as possible.

Description: Based on emergency plan review it was determined the plan was not up to date. Evidence: The emergency preparedness plan listed personnel who are longer at the center as the emergency officer/contact.

Plan of Correction: The center will update the emergency preparedness plan.
