Discussion of an incident that occurred on Monday, May 14, 2018 at approximately 5 pm.

The following unannounced complaint inspection took place on 05/22/2018, between the hours of 10:30 am through 12 pm, on site at the child day center located in Falls Church, Virginia. Description: Based on interview, a child under 10 years of age was not always within actual sight and sound supervision of staff. Evidence - 1. Staff #1 stated (during an interview conducted on the date of inspection - 05/22/2018) that on Monday, 05/14/2018 at approximately 5 pm, a 21 month old child (Child #1) was left unsupervised and alone on the center playground for 4 minutes. 2. Staff #1 also stated that video surveillance of the incident showed Staff #2's failure to account for all of the children in her care when the children left the playground and headed back into the center. Child #1 was viewed still playing on the playground. 3. After approximately 4 minutes, the Parent of Child #1 found the child alone on the playground with the gate to the playground having been left open. 4. This same standard was cited previously during an inspection that took place on 05/01/2018 involving a different age group.

Plan of Correction: Staff #2 was terminated from employment, and all existing staff were retrained on center policy involving transitions from the classroom to the playground and back again. The center will also implement a name to face policy in addition to the existing head count already being done. An e-mail was sent last week to all parents; reminding them of back to school night, where this incident will be addressed and questions answered.
