Technical Assistance:
The following was discussed with the assistant director: *background checks *USDA

Comments:
An unannounced monitoring inspection was conducted from 9:15am-11:30am. Upon arrival there were 67 children in care with 13 staff present. A sample of 3 staff and 6 children's records were reviewed in addition to 3 medications. Children were observed doing worksheets with staff, playing in learning centers, practicing for a play and on the floor playing with staff. The playground areas with the train was currently not open for children to use and will open after the train has been repaired and gumballs have been removed.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure areas and equipment of the center were maintained in a safe condition. Evidence: In the bathroom in the 2 year-old classroom (polywogs) the metal hardware covering the plumbing of the toilet was rusted. Crumbles of rust was observed on the surface and was in reach of children.

Plan of Correction: The following corrective action plan was provided by the assistant director during the exit interview: The rusted hardware will be replaced by the end of the week.

Standard #: 22VAC40-185-290-3
Description: Based on observation the center did not ensure electrical outlets had protective covers. Evidence: In the 3/4 year-old classroom (seahorses) there were 2 electrical outlets on a powerstrip located on the wall above the shelf that did not have protective covers.

Plan of Correction: The following corrective action plan was provided by the assistant director during the exit interview: The staff placed covers on the electrical outlets during the inspection.

Standard #: 22VAC40-185-440-H
Description: Based on observation the center did not ensure all cribs met all requirements. Evidence: In the infant room (monkeys) 3 of the 4 cribs had more than 1 inch of space between the mattress and the crib.

Plan of Correction: The following corrective action plan was provided by the assistant director during the exit interview: The assistant director will call the crib manufacturer to discuss the mattress.
