Thank you for your time and assistance with the unannounced monitoring inspection conducted on June 4, 2018. The on-site inspection was from 1:15p.m. to 2:05p.m. and from 3:45p.m. to 5:30p.m. On this date, there were 23 children present and in the care of six staff members. There was one additional staff member available to assist if needed. There were 46 children enrolled in the center. Five staff members' records and five children's records were reviewed and three classrooms were observed. Four staff members and three children were interviewed. There was no medication on-site or being administered. Afternoon snack met the USDA guidelines and consisted of goldfish crackers and apple juice. The previous violations had been corrected. The exception to compliance was noted on the violation notice and the risk rating was discussed. Some items reviewed/observed: updated liability insurance, annual health and fire inspections, warm water temperature, required posted information, injury reports, documentation of evacuation, shelter-in-place drills and lockdown drills, infant care, diapering changing procedures, hand washing and bathroom procedures, nap time, afternoon snack, and dismissal procedures. Standard #: 22VAC40-185-140-A Description: Based on review of five children's records and verification with the director, the center failed to obtain documentation that one child had received a physical examination within one month of the first date of attendance. Evidence: 1. The record for Child #4 contained a physical examination that was dated 01/06/2017 and date of enrollment for the child was 07/22/2016. 2. The director confirmed that the documentation of the physical examination was not obtained within a month of enrollment. Plan of Correction: The center is aware of this regulation and from now on they will ensure that physical examinations are received either prior to enrollment or within one month of enrollment.
