Comments:
A Subsidy Health and Safety inspection was conducted on June 5, 2018 to review supplemental health and safety requirements. Ten staff were present with fifty-five children, ranging in age from infants through five years. Additional staff were on site for administrative duties. Five children and five staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt.

Description: Based on review, facility did not ensure 1 of 5 staff are certified in CPR and First Aid within 30 days of beginning employment. Evidence: On June 5, 2018, the record for staff #5 (start date April 17, 2018) is not CPR and first aid certified.

Plan of Correction: Staff person has been enrolled in First Aid/CPR/DHO training and required to attend. We will ensure the staff are enrolled in the next available class upon Day 1 training and ensure they understand that this is mandatory training.
