An unannounced monitoring inspection was conducted on June 5, 2018 from approximately 9:30 am to 12:45 pm. Ten staff were present with fifty-five children. Additional staff were on site for administrative duties. Five children and five staff records were reviewed. No medication is currently on site to be administered to children. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt.

Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 3 of 5 children's records have documentation of required information. Evidence: On June 5, 2018, the following was found during record review: 1. The record for child #2 did not have physician's name, physician's phone number and the address for the second emergency contact. 2. The record for child #3 did not have the address for the second emergency contact. 3. The record for child #4 did not have the address for two emergency contacts. Plan of Correction: Parents have been notified to bring the missing information by the close of business. Going forward we will work with parents to ensure all required items are submitted and reviewed at the time of enrollment prior to child's start date. Children files will also be reviewed monthly to ensure proper updates are made.
Standard #: 22VAC40-185-320-B Description: Based on observation, facility did not ensure required supplies are available in each bathroom. Evidence: On June 5, 2018, the bathroom in the Pre-K room did not have toilet paper and paper towels. Plan of Correction: Toilet paper and paper towel were placed in the bathroom before agent left the facility. Staff will be instructed to check all bathrooms in the morning and during nap time to ensure all items are replenished promptly.
Standard #: 22VAC40-185-560-F Description: Based on review, facility did not ensure meals or snacks served follow the most recent USDA requirements. Evidence: On June 5, 2018, the posted menu stated fresh fruit and water were to be served for the AM snack. The menu stated yogurt and water were to be served for the PM snack. USDA requires two of the four components be served for snack. The food components are milk, fruit/vegetable, grains/bread and meat/meat alternate. Plan of Correction: Menu has been updated with the proper components for USDA guidelines. Future menus will be reviewed by management prior to posting.
