Standard #: 22VAC40-185-340-F
Description: Based on staff interviews and statements, a child under 10 years of age was not within actual sight and sound supervision of staff. Evidence: The children came inside from the playground, staff shut the door leaving child #1 out of actual sight and sound. Staff realized child was not in classroom and opened the door. Based on the electronic check in/out system, it was determined the child was left outside for approximately less than one minute.

Plan of Correction: A new way of checking children in/out electronically was been put into effect immediately.
