A Subsidy Health and Safety inspection was conducted on 6/11/18 to review supplemental health and safety requirements. Today's visit was a renewal inspection due to the issuance of a Provisional License. The inspection began at 10:00am and concluded at 2pm. There were thirty-five children observed in care ranging in age from four months through nine years of age with nine direct care staff and the Program Director was present. During the inspection outdoor play was observed, the playground was viewed, infant care was observed, emergency supplies were viewed, three out of eleven staff records were reviewed, five out of thirty-five children's records were reviewed, lunch time was observed, nap time was observed, and some policies and procedures were reviewed. Your renewal application was given the the Licensing Inspector during the time of the inspection. A background check violation was cited and follow-up with your licensing inspector is required to take place within ten days or by 6/21/18. Once today's inspection and the renewal application are deemed complete a recommendation regarding your license can be made. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-012-E Description: Based on the inspector's review of three out of eleven staff records, the center failed to ensure a staff member had completed CPR and first aid training within their first thirty days of employment. Evidence: The record for staff member #2 has a documented date of hire as 4/16/18 and the staff member does not have documenation of the completion of first aid and CPR training. The staff member has been employed for more then thirty days. Plan of Correction: The staff member is scheduled for CPR and First Aid training later this month. The first class the staff member was supposed to attend was canceled.
