Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI - phone number: 1 (804) 726-7884), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and board officers must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.
The following inspection took place between the hours of 10 am through 2 pm. Today, I met with the Staff of the Goddard School located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 108 children with 16 staff; good ratios in all 9 rooms. A total of 10 children?s files and 10 staff files were reviewed. The children were engaged in the following: center activities within their classroom, art, games, story time, writing their name, shapes, circle time, diapering, infant feeding, lunch, hand washing, large motor activities on center playgrounds, nap time, and other organized activities. Standard #: 22VAC40-185-140-A Description: Based on review, 1 of 10 children's files reviewed indicated that each child did not have a physical examination by or under the direction of a physician within one month after attendance. Evidence - 1. On the date of inspection (06/13/2018), documentation of a physical examination for Child #3 was not available for review. 2. Based on the information on file, the start date for Child #3 was listed as - 05/01/2018. Plan of Correction: The parent of Child #3 shall be contacted and documentation of a physical shall be obtained.
Standard #: 22VAC40-185-60-A Description: Based on review, 4 of 10 children's files reviewed did not contain all of the required information. Evidence - 1. On the date of inspection (06/13/2016), Child #8 had listed complete information for 1 of 2 emergency contacts. 2. The following did not have the address for each of their listed emergency contacts: Child #3, Child #5, and Child #6. Plan of Correction: All missing information shall be obtained and placed in the applicable child's file.
Standard #: 22VAC40-185-240-D-1 Description: Based on observation and interview, there was not a staff member who has satisfactorily completed a training program ( for this purpose approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine, or pharmacist) certifying them to administer an Auvi-Q (epinephrine injection, USP auto-injector). Evidence - 1. On the date of inspection (06/13/2018), a total of six (6) Auvi-Qs were observed on site at the center. 2. The Center Director stated (during an interview conducted on the date of inspection) that currently no one was certified on site who as successfully taken the Auvi-Q training. Plan of Correction: Staff will sign up to complete this course.
