An unannounced monitoring inspection was conducted from 11:00am until 2:00pm. At the time of the tour, there were 62 children in care with 11 staff present. A sample of 6 children's records and 7 staff records were reviewed. Children were observed eating lunch, participating in story time, engaging in free play and resting quietly during nap time. Diapering, hand washing and restroom procedures were also observed. Medication, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the area of administration are identified on the violation notice and were discussed with the center director during the exit interview.

Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center did not ensure that each child have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. Child #1 has a date of enrollment of 4/21/18 and was in care at the time of the inspection. The physical located in child #1's record is dated 6/5/18, which is more than one month after the child's attendance. 2. The center director confirmed that the physical for child #1 was completed more than one month after the child's attendance. Plan of Correction: The center responded with the following: Management has been retrained on how to conduct a paperwork appointment with all new enrollments to ensure that all required documentation is on file prior to a child?s start. DM will be conducting quarterly file audits to ensure compliance. The missing physical form was requested from the parent on 6/11/18.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center did not ensure that children's records contain all the required information. Evidence: 1. The records for child #1 and child #2 do not contain the addresses for the two designated people to call in an emergency if a parent cannot be reached. 2. The center director confirmed that there are no addresses listed for the emergency contacts for child #1 and child #2. Plan of Correction: The center responded with the following: Management has been retrained on how to conduct a paperwork appointment with all new enrollments to ensure that all required documentation is on file prior to a child?s start. DM will be conducting quarterly file audits to ensure compliance. The missing information for these two children were requested on 6/11/18.
