Comments:
The Licensing Inspector conducted an unannounced investigation on 5/15/2018 from 8:15am - 11:00am, and on 5/24/2018 from 8:00am to 11:10am in response to a complaint that was received by the licensing office on 05/10/2018 regarding behavioral guidance and ratios. Based upon staff interviews, records reviewed, and observations made, the preponderance of evidence gathered during this investigation does support the allegations. Complaint related: Yes
Description: Based on record review and interviews with staff, the center did not ensure the required ratio of 1 staff person for every 4 children from birth to age 16 months.

Evidence:
According to the documentation provided by the center, entitled, "Child Supervision Record (NAME-TO-FACE)", on 05/08/2018 the infant room was out of ratio from 8:32am to 8:45am with 1 staff member and 5 infants.

Plan of Correction: Will ensure that the 1:4 ratio is maintained at all times in the infant classroom.

Description: Based on record review and interviews with staff, the center did not ensure the required ratio of 1 staff person for every 5 children for children 16 months to 2 year-old.

Evidence:
According to the documentation provided by the center, entitled, "Child Supervision Record (NAME-TO-FACE)", on 05/23/2018 the toddler's classroom was out of ratio from 9:05am to 9:12am with 1 staff member with 6 children.

Plan of Correction: Will ensure that the 1:5 ratio is maintained in the toddler's classroom at all times.

Description: Based on record review and interviews with staff, the center did not ensure the required ratio of 1 staff person for every 8 two year-old children.

Evidence:
According to the documentation provided by the center, entitled, "Child Supervision Record (NAME-TO-FACE)", on 05/23/2018 the two's classroom was out of ratio from 8:40am to 8:50am with 1 staff member with 9 children.

Plan of Correction: Will ensure that the 1:8 ratio is maintained in the two's classroom at all times.

Description: Based on observation, the center did not provide opportunities for school-age children who attend the before school program to do homework or projects or hobbies in a suitable area. Evidence: On 05/15/2018 from 8:15am until 8:45am, and on 05/24/2018 from 8:05am until 8:45am, 4 school-age children attending the center's before school program were observed using the two year-old classroom. During this time there were also 4 two year old children present in the classroom, with 1 staff member. Based on observation, the two's classroom does not provide a suitable area to support school-age children's developmental needs.

Plan of Correction: The school-age children who attend the before school program are now using their school-age classroom, and not the two year-old classroom, where they can do homework, projects, and different activities are provided.

Description: Based on interviews, a staff member did not use behavioral guidance that was constructive in nature, and age and stage appropriate to redirect a child to appropriate behavior.

Evidence:
Staff#1 and staff#2 observed staff#3 using a harsh tone, and making a spitting sound with their mouth when redirecting a child in the infant's room.

Plan of Correction: Teacher was retrained on OP-300 Guidance, Protection and Resect for Children.

Description: Based on observation, material and equipment available were not age and stage appropriate for all children in care. Evidence: During the inspection on 05/15/2018 from 8:15am until 8:45am, and on 05/24/2018 from 8:05am until 8:45am, school-age children were observed using the two's classroom during the center's before school programming time. Based on observation, the materials and equipment in this classroom were not age and stage appropriate for school-age children. The chairs and tables in the two's classroom are sized for children two years of age, the equipment and games consisting of puzzles, small manipulative toys, books, and social living equipment are adequate equipment and materials for two year old's, but are not suitable for school-age children.

Plan of Correction: The school-age children attending the before and after school program are using the school-age classroom and not the two's classroom which was previously used. The school-age classroom is fully equipped with all the materials and resources needed.
