22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal inspection was conducted today from 9:50am-1:30pm. There were 20 children (infants-5yrs.) directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, writing letters, practicing letter sounds, coloring, and eating lunch. Today's lunch consisted of chicken tenders, Marinara sauce, breadstick, baby carrots, apples and milk. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, children's records are missing required information. Evidence: The record of child#1 is missing the father's work telephone number. The record of child#2 is missing the street number for 1 emergency contact. The record of child#3 is missing the mother's work telephone number. Plan of Correction: We will ask the parents for the information.
Standard #: 22VAC40-185-210-A-4-a Description: Based on staff record review, the center did not obtain documentation of qualifications for staff. Evidence: Staff#2, #3 and #4 did not have documentation on file to demonstrate that they meet program leader qualifications. Plan of Correction: We will work on the documentation needed.
Standard #: 22VAC40-185-290-3 Description: Based on observations, unused electrical outlets were missing child-protective covers. Evidence: In the Senior's classroom, one electrical outlet was observed without a child-resistant protective cover. One electrical outlet was also observed missing a child-resistant protective cover under a shelf by the exit door. Plan of Correction: We will cover all outlets.
