An announced record review was conducted on June 13, 2018. Licensing Inspector reviewed Staff Qualifications and Training, Administration, Background Checks and Licensure and Registration Procedures. The sample size consisted of seven staff records. The areas of noncompliance have been identified on the Violation Notice.
Standard #: 63.2-1720.1-B-3 Description: Based on record review, in one of ten staff records reviewed, the licensee did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: During the inspection conducted on June 13, 2018, a Sworn Statement or Affirmation form for Staff #3 (date of hire 8/21/17) indicated that the staff member has lived outside the State of Virginia within the last five years and the center had not obtained an Out of State Child Abuse and Neglect Search Request for that state. Plan of Correction: The person in charge contacted the state while the inspector was present and has already started the process.
