Background check procedures were discussed with the director during the inspection. All staff must complete fingerprint checks by September 30, 2018.
An unannounced monitoring inspection was conducted from 10:35am until 1:30pm. At the time of the tour, there were 54 children in care with 11 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed playing outside, participating in morning learning activities, eating lunch and resting quietly during nap time. Diapering, hand washing and toileting procedures were also observed. Medication, first aid and emergency supplies, documentation of emergency practice drills, children's incident reports and center postings were reviewed. Violations in the area of special care provisions and emergencies are identifed on the violation notice and were discussed with the center director during the exit interview.
Standard #: 22VAC40-185-510-C Description: Based on a review of medication, the center did not ensure that procedures for administering medication shall include methods to prevent use of outdated medication. Evidence: 1. There was an EpiPen Jr. with an expiration date of 5/18/18 and an albuterol inhaler with an expiration date of 3/18/18 in the medication lock box. 2. The center director confirmed that the two medications were expired and had not been returned to the parent. Plan of Correction: The center responded with the following: All expired medication has been returned to parents.
Standard #: 22VAC40-185-510-N Description: Based on a review of medication, the center did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Evidence: 1. Child #1 had an asthma action plan and parent authorization that expired on 10/25/17 and was not renewed. The albuterol inhaler, with an expired prescription date of 4/25/18, was not returned to the parent within the 14 day required time frame. 2. The center director confirmed that the parental authorization was expired and that the albuterol inhaler had not been returned to the child's parent. Plan of Correction: The center responded with the following: Medication will be audited weekly to make sure all medication is updated and current.
